Many events can happen in a person’s life and it may mean you or a loved one needs an extra pair of hands at home. We can respond quickly by providing an understanding and caring support worker. For most people, staying independent and at home with support from carers is preferable to moving into residential care. Because we are so flexible, the type and frequency of visits are tailored to each individual, and those arrangements can be easily altered to meet changing needs.

The first step to setting up successful home care and support is a free no obligation visit to the person’s home. Our visit allows us to introduce ourselves and answer all your questions. This initial home visit helps us get to know your relation, to find out about personal preferences, interests and lifestyle as well as any health related requirements. We will gather this information to get a realistic idea of the level of care that is required & conduct a care and risk assessment for you. Following our visit a detailed plan of care will be drawn up covering all eventualities and a fully costed care package will be offered. We are happy to talk to you, the rest of the family or other care or health professionals as required; but ultimately our services are designed around you, your needs, wishes and expectations.

Personal Care at Home

If you are an older person or you have a disability, you may need some help maintaining your personal care. As we age some of the things we used to do easily have now become much harder. It may be because your health or mobility is declining, or you have been in hospital or you have a long term health condition.

You may find getting washed and dressed more challenging in the mornings or you need help to get into your nightwear in the evenings. You may need help with continence, going to the toilet, brushing your teeth, your hair or even having a shave. We can assist you as much or as little as your require.

Medication Care at Home

Support Me at Home are here to help you or your loved one with medication assistance. We can make sure you are following your medication schedule properly and on-time. You may find it difficult due to having multiple medical problems or experience declines in vision, grip strength or memory.

In these cases, playing an active role in managing your medications becomes even more necessary for your safety and health. Doing so will also help give you peace of mind.

Age-related changes also affect the way certain medications are absorbed by and work in the body. We can make sure you get the most from your medications & make sure they are taken the right way. Our support workers are fully trained and competent to reflect safe medication practices within Domiciliary Support Services.

Dementia Care at Home

We work closely with families and advocates to build a real picture of the person we are looking after and ensure that each part of the history we learn about our service users, helps us build a care and support plan that reflects all of who they are and want to be. We ensure that each service user has specific carers assigned to their daily needs and ensure the same carers are available to our service users and their families. This helps not only with continuity of care and building of rapport with our service users, but also with family relationships.

We are currently organising Dementia Champions amongst our staff and are investing both time and money in specialist dementia training for our teams who are integral to creating meaningful, enjoyable days in the lives of our service users with dementia. A Dementia Friends Champion is a volunteer who encourages others to make a positive difference to people living with dementia in their community. They do this by giving them information about the personal impact of dementia, and what they can do to help.

Sitting Service

This service covers those that need sitting services for various reasons. Our services are available in North Norfolk. We are available for short or long term needs.

Older people can often experience extreme isolation from their community and the outside world. Communication can become frustrating and difficult too when sight and hearing is impaired. Our services aim to relieve this and provide regular contact and support. Our services can be a means of accessing community services, enabling participation in activities and helping to maintain independence. We believe every individual should have the support they need.

This service is also available to give family carer’s a break. You may be looking after a spouse but are unable to leave them on their own while you have a break, go to the shops or attend an appointment. Support Me at Home works alongside CAP – The Carer’s Agency Partnership – who are funded by Norfolk Council to help pay for a sitter & give you the time you need.

Help Around the Home

Our practical home help service assists those who need help with day to day house work & laundry. We offer practical home help for the elderly, those with disabilities, with mental illness, and anyone who struggles to get practical tasks done in the household. From assisting with putting the washing on and hanging it out to cleaning your bathroom, kitchen and floors. Any job you find difficult, we can help you with.

Our Shopping Service is operated from Monday to Friday. We will use the local supermarkets in your area. Anyone can use our shopping service. If you need help we can do it for you. All you need to do is give us a ring and we will come to your house and collect your shopping list. The shopping is delivered on an agreed day on either a weekly or fortnightly basis, depending on your requirements. If you require help with writing your list we can easily help with that too. We will deliver it to your door at a time requested by you, we can also help you put it away in the cupboards if you want help.

Other Homecare Services

No two people are the same so that means Support Me at Home offers a variety of services to meet service user’s different needs. Some people may require help with washing and dressing, some may not, but whatever support you need at home you can rely on Support Me at Home to provide the care and service you require.

Additional Services:- We also provide additional services as and when required. For example we do a pop-in service to change Catheter Leg Bags. So if there is a service that you require that is not listed on this page then just give us a call. We are more than happy to provide any service that helps you or a loved one remain independent and at home.
